Why does fluid build up in the chest?
A pleural effusion refers to the build-up of fluid in the pleural space – the area between the lungs and the chest wall. Lung cancer
A common cause of pleural effusion
In most cases, a pleural effusion is caused by a malignant tumor, particularly lung cancer. This type accounts for a significant proportion of all cases. Cancer cells can spread in the area around the lungs – especially to the pleura – where they may block the normal drainage of lymphatic fluid. At the same time, tumor cells can release substances that make blood vessels more permeable. As a result, fluid leaks into the pleural space.
This is known as a malignant pleural effusion. The fluid is often rich in protein, may appear bloodstained, and can contain cancer cells when examined. Especially in advanced or non-operable cancers, pleural effusion is a common associated symptom.
Common associated conditions:
Shortness of breath that worsens as fluid levels rise
Effusion on one side, usually on the same side as the tumor
Rapid recurrence following a puncture
General fatigue, cough, chest tightness

Herzinsuffizienz
Flüssigkeit durch Rückstau aus der Lunge
Bei einer Herzinsuffizienz – oder auch Herzschwäche genannt – kann das Herz das Blut nicht mehr ausreichend durch den Körper pumpen. Dies führt zu einem Rückstau in den Lungengefäßen und zu einem Druckanstieg in den Kapillaren der Lunge. Als Folge davon wird Flüssigkeit durch die Gefäßwände in den Pleuraspalt gedrückt.
Dieser Ergusstyp ist in der Regel ein sogenannter transsudativer Pleuraerguss, das heißt: Die Flüssigkeit ist klar, eiweißarm und zellarm – im Gegensatz zum entzündlichen oder tumorbedingten Erguss.
Typische Anzeichen:
Beidseitiger Erguss, oft rechts stärker
Schwellungen an den Unterschenkeln (Ödeme)
Gewichtszunahme durch Wasser, nicht durch Fett
Atemnot, vor allem im Liegen oder bei Belastung
Herzinsuffizienz ist eine chronische Erkrankung. Das Auftreten eines Pleuraergusses ist ein Hinweis darauf, dass sich der Zustand verschlechtert. Die Behandlung besteht in erster Linie aus Herzentlastung – durch Medikamente, Ernährung und Bewegung. Wenn trotz Therapie Ergüsse immer wiederkehren, kann auch hier ein Dauerkatheter eine sinnvolle Lösung sein.

Heart failure

In heart failure – also known as a weakened heart – the heart is no longer able to pump blood effectively throughout the body. This can lead to a build-up of pressure in the blood vessels of the lungs, causing fluid to be pushed through the vessel walls into the pleural space.
This type of effusion is usually what is known as a transudative pleural effusion. This means that the fluid is typically clear and contains low levels of protein and cells, in contrast to effusions caused by inflammation or cancer.
Common signs:
Fluid accumulation on both sides, often more severe on the right
Swelling in the lower legs (edema)
Weight gain due to water, not fat
Shortness of breath, especially when lying down or during physical activity
Heart failure is a chronic condition. The development of a pleural effusion can be a sign that the condition is worsening. Treatment primarily focuses on reducing the strain on the heart – through medication, diet, and physical activity.
If pleural effusions continue to recur despite treatment, an indwelling catheter may also be a suitable option in these cases.
Breast cancer
When tumor cells spread to the chest
Breast cancer is also one of the more common causes of malignant pleural effusion, particularly in patients with advanced disease or metastases. Cancer cells can spread to the chest area through the lymphatic system or bloodstream and settle on the pleura.
In these cases, the effusion is often the result of pleural carcinomatosis – a widespread involvement of the pleura by cancer cells. The fluid may build up again within a few days, even after it has been completely drained.
For many people, the main focus is on long-term relief of symptoms such as shortness of breath, a feeling of tightness, or pain. The placement of an indwelling catheter can allow for regular, comfortable drainage at home – without the need for repeated hospital stays.
Characteristic features of a breast cancer-related effusion:
Rapid accumulation of fluid, often despite ongoing treatment
A feeling of pressure and tightness in the chest
Loss of appetite, fatigue, and weakness
Fluid usually builds up on one side of the chest, often on the same side as the condition
Andere Tumore
Wenn der Krebs aus dem Bauchraum in die Pleura gelangt
Neben Lungen- und Brustkrebs können auch andere bösartige Tumorerkrankungen einen Pleuraerguss verursachen. Besonders häufig treten Ergüsse bei Eierstockkrebs (Ovarialkarzinom), Bauchspeicheldrüsenkrebs (Pankreaskarzinom), Magenkarzinomen und Leber- oder Darmkrebs auf.
Diese Tumore streuen oft über das Bauchfell (Peritoneum) oder über die Lymphbahnen. Im fortgeschrittenen Stadium können sie sich bis zur Pleura ausbreiten oder dort den Lymphabfluss behindern. Auch bei diesen Tumoren sind wiederkehrende Ergüsse ein häufiges Symptom – verbunden mit erheblichen Belastungen im Alltag.

Kombination aus Aszites (Bauchwasser) und Pleuraerguss

Other tumors

In addition to lung and breast cancer, other types of cancer can also cause a pleural effusion. These are particularly common in ovarian cancer, pancreatic cancer, stomach cancer, and cancers of the liver or intestines.
These cancers often spread through the abdominal lining (peritoneum) or the lymphatic system. In more advanced stages, they may reach the pleura or interfere with the normal drainage of fluid. Recurrent pleural effusions are also common in these cases and can place a significant burden on daily life.

Cancer with known metastases
A combination of ascites (fluid in the abdomen) and pleural effusion
Low physical endurance
Repeated punctures with only short-term relief

Benign causes
It's not always caused by a tumor
Around a quarter of all pleural effusions are not caused by cancer, but by what are known as benign (non-cancerous) conditions. Even in these cases, a significant build-up of fluid can occur – often with similar symptoms.
Common benign causes:
Liver cirrhosis with ascites:
Fluid can pass through the diaphragm into the chest, usually on the right side.
Heart failure:
Caused by a build-up of pressure in the blood vessels of the lungs.
Kidney disease:
Significant loss of protein can lead to fluid shifts in the body.
Inflammatory conditions:
For example, pneumonia or tuberculosis
Autoimmune diseases:
Such as lupus erythematosus or rheumatoid arthritis
Pulmonary embolism:
A blockage of a blood vessel in the lungs caused by a blood clot.
In these cases as well, a thorough diagnosis is needed – including imaging tests and examination of the fluid (thoracentesis). Treatment depends on the underlying condition, but if the effusion keeps returning, an indwelling catheter may also help relieve symptoms.
